What are the CDC rules for bringing a dog into the US?

The short answer

CDC says rules depend on where your dog has been in the last 6 months and where it was vaccinated. For dogs only in rabies-free or low-risk countries, the CDC Dog Import Form is the only required document, plus a microchip and a minimum age of 6 months.

Dogs from rabies-free or low-risk countries

Since August 1, 2024, the only required document for these dogs is the free CDC Dog Import Form. The receipt is valid for 6 months, and can be used for multiple entries if the dog has not been to a high-risk country.

CDC says each dog needs its own form, and you can show the receipt printed or on your phone.

Dogs from high-risk countries

For a U.S.-vaccinated dog that has been in a high-risk country, CDC requires two documents: the Dog Import Form receipt and the CDC Certification of U.S.-issued Rabies Vaccination form, completed by a USDA-accredited vet before departure. This form is not the same as a regular rabies certificate.

CDC says USDA-endorsed export health certificates issued after July 31, 2025 are not accepted for re-entry. The receipt is valid for a single entry, on the arrival date listed.

A dog in a high-risk country that is not vaccinated will not be allowed to enter. Foreign-vaccinated dogs from high-risk countries have extra steps, including a reservation at a CDC-registered animal care facility.

Other rules

CDC says you must also follow USDA and your destination state's rules, and dogs imported for resale have extra USDA requirements. CDC-INFO is (800) 232-4636.
